*Child Protection Training Certificates for ALL counselors and
adults who will be visiting the camp.

*Volunteer Screening Forms for ALL counselors.
Volunteer Screening Form As part of this form, the church is required to do a Sex Offender background
check on each counselor. The free report can be done through the Texas Department of Public Safety.
Please print out the search results and attach to the Volunteer Screening Form.
Pastor Recommendation Letter
*Remaining balance of $130 per person - preferably in one
check made out to Gulf Coast Baptist Association.
*All medication in original containers with names clearly labeled.
